This form can be printed and submited to LSA for ODP enrollment.

LSA Olympic Development Program ( ODP ) Form

LSA Youth Division Request for Player Transfer or Release
This form is a multi-part form which is available from LSA and often your local organization. The sample available on-line CANNOT be submitted. It is used to gain the required approval for a player to transfer or br dropped from a team. The form has 4 parts:

Section A. - Player Information

Section B. - Releasing league/Team Information

Section C. - Accepting League/Team Information ( unused if just a Release )

Section D. - LSA Action section.

INSTRUCTIONS:

1. The player's parent/guaradian completes all items in Section A, then forwards the request to current league president/registrar. A $2.00 Transfer Fee must accompany each request. Make checks payable to LSA.

5. A player wishing to drop from the program and NOT transfer to another team, should complste Section A and forward to the league for completition of Section B and forwarding to the LSA office. No fee is required for this action
